New student / transferee checklist

  • Original Report Card (SF9)
  • PSA Birth Certificate
    • Clear colored photocopy of the Birth Certificate issued by the Philippine Statistics Authority (PSA)
    • If the PSA copy is not legible, submit a legible original copy from the Local Civil Registrar (LCR) together with the PSA photocopy.
  • Recent I.D. picture (2×2)
  • Intake interview with the Guidance Advocate (by appointment)
  • Scanned copy of Good Moral character document
