Admission Requirements for New Student/Transferee:

• Original Report Card (SF9)
• PSA Birth Certificate
  • Clear colored photocopy of the Birth Certificate issued by the Philippine Statistics Authority (PSA)
  • If the PSA copy is not legible, the applicant must submit, in addition to the clear photocopy of PSA issued Birth Certificate, a legible original copy of the Birth Certificate issued by the Local Civil Registrar (LCR)
• Recent I.D. Picture (2X2)
• Intake Interview with the Guidance Advocate (by appointment)
• Scan Copy of Good Moral
